Filter Skilled Nursing Facilities by their cost report: report details, days, beds, discharges and admissions, SNF and NF statistics, salaries and costs, assets, liabilities, and revenue and income.
A Skilled Nursing Facility is a healthcare facility that provides nursing care and rehabilitation for people who need medical support beyond what can be provided at home. SNFs help patients recover from illness, surgery or other medical conditions, and support residents who need ongoing care.
In CMS terms, a SNF participates in Medicare and an NF participates in Medicaid, and many facilities are certified for both. The Program Participation Code filter lets you separate them, with options such as SNF (Medicare) Only, NF (Medicaid) Only and SNF/NF Dual Certification. Provider Type in Provider Information gives a similar Medicare, Medicaid, or Medicare and Medicaid split.
Provider Information includes the overall rating, health inspection rating, QM rating, long-stay and short-stay QM ratings and staffing rating, each with a minimum and maximum filter. MDS Quality Measures and Medicare Claims Quality Measures let you go a level deeper, to individual measures and their scores.
It holds facility results from the Skilled Nursing Facility Value-Based Purchasing program. You can filter by program ranking, performance score and incentive payment multiplier, and by four measures: readmission (SNFRM), healthcare-associated infection (SNF HAI), total nursing staff turnover and total nurse staffing. Each has baseline FY 2022 and performance FY 2024 results, plus achievement, improvement and measure scores.
Yes. Inspection Dates and Survey Summary cover survey dates, cycles and total deficiency counts, including counts by deficiency type. Fire Safety Deficiencies and Health Deficiencies go to the record level, with deficiency category, tag number, scope severity code, correction status and correction date.
Footnotes explain why a result is missing or qualified, for example because there was not enough data to calculate a result or the facility did not meet the case-minimum policy requirement. Filtering on footnotes helps you tell a low score apart from a score that could not be calculated or publicly reported.
Yes. The Ownership section filters by owner name, owner type, ownership role, ownership percentage and association date. Provider Information adds the number of facilities in a chain, chain average ratings, and whether the provider changed ownership in the last 12 months. Each profile also lists the current owners on file.
PBJ Employee Detail Staffing filters by recorded work hours. Provider Information covers reported staffing hours per resident per day for nurse aides, LPNs, RNs and physical therapists, weekend staffing, case-mix and adjusted staffing hours, and nursing staff, RN and administrator turnover.
Yes. Enter an EHR vendor name in the EHR Stack search bar to find facilities associated with that vendor. Each profile's Technology Stack section also shows the technologies and vendors associated with the facility. Technology data reflects what has been identified and may not be a complete inventory.
Financials filters cover latest funding type, annual revenue, total funding and latest funding amount. The SNF Cost Report adds detailed cost report fields: days, beds, discharges and admissions, salaries and operating costs, assets, liabilities and fund balances, and revenue and income.
